Xen users can use ConVirt 1.1 GUI to add a new disk to a running VM.

For adding a new disk, select a running VM and click Change VM Settings. Click the Add button in the Connected Disk section. A new dialog will popup where you can specify the disk parameters. The new disk would get added using xm attach to the running VM when you save the changes.
