Sometimes when adding an instance in CCP 3.0.6 using Chrome(Version 26.0.1410.64m), click the Launch VM button closes the wizard, but nothing else happens - no VM is created, nothing appears in the logs or on the Instance screen.

Enabling the Chrome debug reveals a response: 505 (HTTP Version Not Supported) when clicking Launch VM, here is the full debug when using the Add Instance wizard:
