In CCP 3.0.6 when scrolling the Templates > ISO screen the last two ISO files repeat continuously as you scroll down the screen (see attached screenshot). The repeating ISO files have the same ID value. The CCP database contains only a single entry for each of these ISO files, so the problem appears to be with GUI.
It looks to me like some web UI problem. What is your browser?
Can you check in any other browser if you see the same behavior?

This is happening in 3 different browsers: Chrome(Version 26.0.1410.64m), IE9 and Firefox 20.0.1
This is a known issue.
CS-16255 - Duplicate entries populating in ISO list
It's scheduled to be fixed in the upcoming CloudStack "Campo" release.
