When a template is deployed, it is copied from SecondaryStorage to the appropriate PrimaryStorage (if it hasn't already been copied there), then cloned into the new virtual disk.

I've run into instances where the template has been removed from one of the PrimaryStorage locations, outside of CCP, and seen errors similar to your.

Any chance that is what is going on?

I"m not really sure just yet.  Our prototype is setup so that both primary and secondary storage reside on the same server.

I always thought templates would be saved on "Secondary" and never moved to "Primary".
